This is a remote, full-time position at 40 hours/week. The core working hours will be between 11:30am-8pm ET, Monday through Friday with a requirement of 1 weekend day coverage per month.

Here’s what you’ll do:

  • Telephonic outreach, including cold calls, to patients for post discharge assessments of clinical symptoms, barriers to medication adherence, safety concerns, social needs
  • Educate and coordinate preventative health screenings
  • Perform chronic disease management and medication adherence education
  • Navigate conversations with patients seeking insight on Galileo’s care model
  • Facilitate the coordination of care between health care services, including hospital/ED care, pharmacies and community providers to improve patient outcomes
  • Develop an understanding of various health plan contracts / goals, Galileo markets, and needs of various patient populations
  • Be accountable to performance targets as an individual contributor
  • Collaborate internally with Engagement and Population Health leadership to improve population outcomes

Physical Requirements:

About the Role

The Registered Nurse Manager is a critical role on the Galileo care team. This individual will lead a team of RNs and collaborate with our multidisciplinary team to ensure our patients receive the highest quality care when they have a clinical concern.

ABOUT THE ROLE

As a Lifecycle Marketing Associateat Galileo, you’ll play a pivotal role in executing user-centric campaigns that improve acquisition, engagement, and conversion through digital owned channels. You’ll own the day-to-day operations of our customer engagement platform, and partner with Lifecycle, Product, and Brand to effectively reach patients with relevant and personalized content.

ABOUT YOU

  • Strong analytical and problem-solving skills, with the ability to use data analysis tools (e.g. Looker, Excel/Sheets) to draw results and insights.

About the Role:

About the role:

As an Engagement Specialist, you’ll have the opportunity to directly solve some of healthcare’s toughest challenges. Here’s what you’ll do:

  • Perform outbound calls to promote Galileo and generate new patient volume. 
  • Field inbound interest resulting from marketing awareness campaigns to guide patient sign-ups and scheduling. 
  • Drive positive outcomes that include, for example, clinical visit scheduling, account activation, and application download.
  • Collaborate with Marketing to shape, test, and iterate messaging.
